Welcome to Stroud Community Services – your local provider of home care services.  Our service supports older people and their carers to live independently at home and remain a part of their community. We provide services for government-subsidised and private self-funded clients.

How do I access services?
Just give us a call at 0493 083428. Our Community Care Coordinator will organise a time to visit you in your home, and discuss your needs – this is free and with no obligation. If we can assist, we will work with you on a plan tailored to your individual circumstances, including a written agreement outlining your services and fees.

If you are already with another provider and would like to switch to Stroud Community Services we can guide you through the process of changing providers.

What levels of support does the government offer for home care?
The Australian Government’s Home Care Packages Program provides a subsidy towards a package of care, services, and case management to meet your personal needs. There are four levels of home care packages.

Level 1 supports people with basic care needs
Level 2 supports people with low-level care needs
Level 3 supports people with intermediate care needs
Level 4 supports people with high-level care needs.

Each level of home care package provides a different subsidy amount which contributes to the total cost of your service and care delivery. If you are eligible for a government-funded home care package, there may be a very small contribution to the cost of your care.

How do I apply for government subsidies for home care?
To apply for government funding for home care services you need to:

  1. Contact the My Aged Care Team on 1800 200 422 to organise an assessment.

    This gives the assessor an idea of what support you might need to help you return to, or keep the level of independence needed to manage your day-to-day life. They’ll also ask about:

    • what support you already have, and if it will continue
    • your health, lifestyle, and any health concerns
    • how you’re going with completing daily tasks and activities around the home
    • if you have problems with your memory
    • any issues relating to the home and personal safety
    • family and community activities
    • speaking to your GP or other health professionals.
  2. Once you have received your assessment regarding your package level give Stroud Community Services a call on 0493 083428. We will have a chat with you and go through your assessment and the services that have been recommended for you.

Can I tailor my care package?

We pride ourselves on Consumer Directed Care and we will actively involve you and your support person to provide the best services suited to your needs. Our friendly and experienced team will come out to your home and discuss all the options available.

Home Care Packages are flexible; there may be other care and services that are suited to your needs. Together with Stroud Community Services, we will work out if a care or service that you need:

  • Is directly linked to your identified care needs and goals.
  • Will improve your health and well-being.
  • Is necessary for you to remain living safely and independently in your home.
  • Can be delivered within your Home Care Package budget, and
  • Would be considered an acceptable use of government funds.
